OFFICIAL FORM(S) REQUIRED:
• DSP 11-application
• DSP 19-amendment
• DSP 64-lost/stolen
• DSP 82-application by mail
You may need to use one or more of the official forms. Each form contains
detailed instructions on it's use. Basically, if you already have a valid
passport you need to fill-out DSP 19 and mail it along with a certified marriage
certificate and your current passport to an address listed on the back of the
form, there is no fee.
Be aware that if your passport has expired you will need to fill-out DSP 82
instead and mail it along with a certified marriage certificate and your most
recent passport. The Passport Agency charges a fee for this. Also, if your old
passport is lost or stolen or if you cannot provide a certified marriage
certificate you may need to use application form DSP 11 and possibly the lost
passport affadavit form DSP 64. In either of these cases you will have to apply
in person at your local Passport Agency.
